10. שינויים בפרטי הבקשה
(א)חלו אחרי מתן רשיון שינויים בפרטים הכלולים בבקשה לרשיון או לחידושו, יודיע עליהם המנהל לשרים תוך חודש מיום השינוי.
(ב)ההודעה תישלח למחלקה לטיפול בנפגעי סמים שבמשרד העבודה והרווחה או למחלקה לטיפול בהתמכרויות שבמשרד הבריאות ולועדה.
