8. מתן רשיון ותוקפו
(א)רשיון יהא ערוך לפי טופס 4 שבתוספת.
(ב)תוקף רשיון למוסד משולב יהיה לשנתיים; השרים רשאים לתת רשיון זמני לתקופה שלא תעלה על שנה אחת.
