תצהיר יהיה ערוך בגוף ראשון, מחולק לסעיפים, ומכיל רק עובדות שהמצהיר יכול להוכיחן מתוך ידיעתו הוא או, אם ציין את הנימוקים לכך, גם לפי מיטב אמונתו.
