הסכמת המועמד לפי סעיף 57(ט) לחוק תהיה לפי הטופס שבתוספת לכללי הבחירות לכנסת (נוסח וצורה של כתב הסכמה של מועמד לכנסת), התשע״ח–2018 והיא תהווה חלק בלתי נפרד מרשימת המועמדים.
