3. צורת הדוח
(א)מנהל התיקים יערוך את הדוח בצורה ברורה ונוחה לקריאה, עמודי הדוח יהיו ממוספרים, ויכולים לכלול בראשם את סמלו המסחרי של מנהל התיקים.
(ב)כותרות הפרקים בדוח יובאו באותיות מודגשות ובהגדלה; כמו כן יודגשו כותרות משנה ופרטים נוספים לפי המתכונת שבתוספת הראשונה.
