1. בקשה לתעודת הכשר
(א)בקשה לתעודת הכשר עבור בית אוכל או יצרן תהיה לפי טופס 1 שבתוספת, ויצויינו בה חמרי הגלם, שמות הספקים ותעודות ההכשר הקיימות.
(ב)בעל בית האוכל או היצרן של המצרך, לפי הענין, יחתמו על הבקשה.
