2. תוכן הערר
(א)בערר יפורטו –
(1)שמו ומענו של העורר ומען להמצאת מסמכים;
(2)התאריך שבו נמסרה לו ההחלטה שעליה מוגש ערר;
(3)העובדות שעליהן מסתמך העורר;
(4)הנימוקים שעליהם מבוסס הערר;
(5)הסעד המבוקש.
(ב)כתב ערר ייחתם ביד העורר או בא־כוחו.
