6. דין וחשבון על עיקול מיטלטלין
(א)גובה המס יערוך דין וחשבון על הטלת העיקול; דין וחשבון כזה יהא ערוך לפי טופס 5 שבתוספת.
(ב)העתק מדין וחשבון העיקול יימסר לחייב או לבן משפחתו הבוגר הגר עמו, ובהיעדרם ישאיר גובה המס העתק מהדין וחשבון במקום נראה לעין במקום שבו בוצע העיקול.
