16. אבחנה רפואית
נקבעה אבחנה רפואית כאמור בתקנה 20(א), ימסור על כך השירות הרפואי המוסמך לועדה הרפואית, למוסד לביטוח לאומי ולמפקח העבודה האזורי, בציון שמו המלא של האדם שנפגע במחלה הקשורה בחומר, מספר הזהות שלו, גילו, מקום עבודתו, תהליך העבודה ומשך תקופת עבודתו בחשיפה לאבק מזיק.
