2. מתן אישור
מנכה שניכה מס הכנסה במקור מתשלום ששילם למקבל, יתן לו בעת הניכוי, לגבי אותו תשלום, אישור שיכלול פרטים אלה:
(א)לגבי המנכה:
(1)שמו ומענו;
(2)מספר תיק הניכויים במס הכנסה;
(3)הסכום שממנו נוכה המס;
(4)סכום המס שנוכה;
(5)תאריך התשלום.
(ב)לגבי המקבל:
(1)שמו;
(2)מספר זיהויו או מספר תיקו במס הכנסה.
