4. פנייה לבקשת סיוע
פעולות הסיוע יתבצעו על פי פנייה מטעם נציג משרד הבריאות לראש אגף בשירות; הפנייה תכלול את שם החולה, תעודת הזהות שלו, מספר הטלפון שלו או חלק ממידע זה.
