11א. תביעה לגמלת ילד נכה
תיקון: תש״ע
(א)תביעה לגמלה בעד ילד נכה תוגש בידי הורהו על גבי טופס שהורה עליו המוסד, ואם הילד אינו נמצא עם הורה – בידי אפוטרופסו ובאין אפוטרופסו – בידי מי שמחזיק אותו בפועל.
(ב)לתביעה כאמור יצורף אישור רפואי, ובו יפורט, בידי מי שקבע לכך המוסד, האבחון הרפואי של הילד והטיפול הניתן לו, או הערכה לגבי התפתחותו ומצבו התפקודי של הילד.
