תיקון: תשמ״א
נוכח המנהל כי שמו של רוקח או של עוזר רוקח שונה כדין ונמסרה הודעה כאמור בתקנה 7, יורה לשנות את שמו של הרוקח או של עוזר הרוקח ברשיונו ובכל רישום אחר הנעשה מטעם משרד הבריאות.
