הודעה על הטלת קנס מינהלי שאינו קצוב, כאמור בסעיף 9(א) לחוק, ותשובה לרשם, כאמור בסעיף 9(ב) לחוק, יהיו לפי טופס 5 שבתוספת או הספח שלו, לפי הענין.
