פעולות הסיוע יתבצעו על פי פנייה מטעם נציג משרד הבריאות לראש אגף בשירות. הפנייה תכלול את שם החולה, מספר הזהות שלו, מספר הטלפון שלו, המספר הסידורי שלו, תאריך האבחון והתאריך שבו חל 14 יום לפני מועד האבחון, או חלק מפרטים אלה; משרד הבריאות ישלח לחולה הודעה על כך שנערכת בעניינו בדיקה באמצעים טכנולוגיים.
