3. פנקס מרפאות
(א)המנהל ינהל פנקס מרפאות (להלן – פנקס המרפאות), שבו ירשום את המרפאות שקיבלו תעודה כאמור בתקנה 7.
(ב)המנהל יקבע את צורת פנקס המרפאות ופרטי הרישום בו.
