כל שנוי שחל בפרטים שנרשמו צריך להודיע עליו בתוך ארבעה עשר יום מיום השנוי בטופס שבתוספת הרביעית לתקנות אלה, והרושם יכניס אותו שנוי בפנקס הרישום.
