2. אגרה בעד תעודת רישום
בעד תעודה המעידה על רישום בפנקס כאמור בתקנה 13 לתקנות סיעוד בבתי חולים, תשולם אגרה בסכום כמפורט בפרט 29 בתוספת השנייה לתקנות האגרות.
