15. מתן היתר
(א)המנהל יציין בהיתר פרטים אלה:
(1)כמות התכשירים שהמבקש רשאי לרכוש על פיו;
(2)מטרת השימוש;
(3)מועד השימוש.
(ב)היתר יוצא בשני עותקים.
