16. גמר התמחות
תמה תקופת ההתמחות, יגיש המתמחה למועצה בקשה לאישור על סיום התמחות, בצירוף הודעה מאת המאמן על גמר התמחותו בנוסח לפי טופס 5 שבתוספת; ראתה המועצה כי המתמחה קיים את הוראות תקנות אלה במלואן, היא תיתן לו אישור כמבוקש.
