4. בקשה למסירת מסמך בישראל
תיקון: תשט״ו
(א)בקשה למסור בישראל מסמך שנעשה באחת המדינות בעלות האמנה תוגש על ידי הרשות המבקשת למנהל בתי המשפט ויצורף לה המסמך.
(ב)בבקשה יצוינו: שם הרשות שהיא מקור המסמך המועבר, שמותיהם ותיאורם של הצדדים למסמך, מענו של האדם שהמסמך נועד לו ומהותו של המסמך.
