4. בקשה להמצאת מסמך בישראל
(א)בקשה להמציא בישראל מסמך שנעשה באחת המדינות בעלות האמנה תוגש על־ידי הרשות המבקשת למנהל בתי המשפט ויצורף לה המסמך בשני עותקים.
(ב)בבקשה יצויינו שם הרשות שהיא מקור המסמך המועבר, שמותיהם ותיאורם של הצדדים למסמך, מענו של האדם שהמסמך נועד לו ומהותו של המסמך.
