הסכמתו של מועמד לפי סעיף 35(ח) לחוק, תינתן בטופס מספר 5 שבתוספת, וכן יצורף לה טופס הסכמה, חתום בידו, למסירת מידע מהמרשם הפלילי ומידע על תיקים תלויים ועומדים לפי התוספת השלישית לחוק המידע הפלילי ותקנת השבים, התשע״ט–2019.
