תיקון: תשפ״ג־2
תובענה לקביעת אבהות תוגש לפי טופס 21 שבתוספת הראשונה ויכול שתוגש יחד עם בקשה לצו עריכת בדיקה גנטית לקשרי משפחה לפי חוק מידע גנטי, התשס״א–2000, כאמור בתקנה 15(א)(5), וכן יכול שתוגש בקשה לצו לעריכת בדיקה גנטית בטופס 21, אף בלא תובענה לקביעת אבהות.
