76. בקשה למתן צו אימוץ
(א)בקשה למתן צו אימוץ תוגש בידי המאמץ לפי טופס 11 לתוספת הראשונה או בדומה לו, כשהיא חתומה ביד המאמץ ומאושרת בתצהיר.
(ב)הבקשה תוגש בשלושה עותקים ורשאי בית המשפט לצוות על המאמץ להגיש עותקים נוספים.
