5. הודעה על מען
רופא וטרינר יודיע למנהל, בכתב, על מען מגוריו ועל מען מרפאתו, וכן על כל שינוי בהם, תוך ארבעה עשר ימים מיום שניתן לו הרשיון או מיום השינוי, לפי הענין.
