9. תצהיר
מחזיק מקום ציבורי ישלח למשרד הכלכלה לא יאוחר מיום 31 בדצמבר בכל שנה, תצהיר חתום ומאומת על ביצוע בדיקות תקינות למכשיר ההחייאה כאמור בתקנות אלה, על פי הנוסח המפורט בתוספת הרביעית.
