בקשה זו תכיל את שמו, כתובתו ומספר הזיהוי או פרט אחר שדרש הרשם של האדם התובע כי הוא זכאי בסימן, בצירוף פרטים מלאים על המסמך אשר מכוחו הוא תובע, לכשיש מסמך כזה, ויש להמציא את המסמך לרושם לעיון.
