בקשה להארכת זמנו של רשיון אחרי שבעל הרשיון הגיע לתגלית תוגש בשלושה טפסים ויפורט בה האופן שבו אומר המבקש להגדיר את שדה הנפט ואומד הזמן הדרוש לכך.
