16. תעודת חיים
לאישור של נוטריון שאדם פלוני נמצא בחיים תצורף גם חתימתו של אותו אדם שניתנה בפני הנוטריון; אולם אם נבצר מאותו אדם לחתום, יציין הנוטריון עובדה זו והסיבה לכך באישורו.
