38. בקשה להיתר
(א)בקשה למתן היתר תוגש בכתב למפקח מאת בעל כלי שיט המעוניין להעסיק בכלי שיט את מי שלגביו מבוקש ההיתר בדרגת ההסמכה המבוקשת.
(ב)בבקשה יצוינו פרטים כפי שיורה מפקח.
