2. נטילת דוגמאות
פקיד הנוטל דוגמה של צרכי מזון יתן למחזיק בצרכי המזון שמהם ניטלה הדוגמה (להלן – המחזיק) קבלה בחתימת ידו שתכיל פרטים אלה:
(1)השם והמען של המחזיק;
(2)מקור הדוגמה, טיבה וכמותה;
(3)תאריך נטילת הדוגמה;
(4)שמו, תארו, תפקידו וחתימתו של הפקיד נוטל הדוגמה.
