3. הגשת בקשה
(א)הרוצה בהסמכה כאמור יגיש למזכירות המועצה בקשה בכתב בה יבקש להרשות לו לגשת לבחינות או לפטרו מהן.
(ב)לכתב הבקשה יצורפו מסמכים להוכחת העובדות המפורטות בתקנה 1(1), (2) ו־(3) וכן תיאור תולדות חייו ושתי תמונות של המבקש.
