3. טפסים
תיקון: תשע״ב־2
(א)נקבע בחוק או בתקנות שיש להגיש בקשה או הודעה בטופס, לא יוגשו הבקשה או ההודעה אלא בטופס שנקבע לאותו ענין בתוספת הראשונה.
(ב)לא נקבע טופס כאמור בתקנת משנה (א), יוגשו הבקשה או ההודעה בכתב בצורה שנקבעה להגשת מסמכים לרשות, ויצויין בהם שמו של מגיש המסמך ומענו למסירת מסמכים בישראל.
