12. בקשה בידי האפוטרופוס
(א)לא הגישו ההורים המיועדים לבית המשפט בקשה למתן צו הורות בתוך המועד שנקבע, תוגש בקשה כאמור בצירוף תסקיר, בידי האפוטרופוס, באמצעות בא־כוח היועץ המשפטי לממשלה, בהקדם האפשרי ולא יאוחר מ־21 ימים מיום לידת הילד.
(ב)ההורים המיועדים יהיו משיבים בבקשה.
