2. הכנת תיק
החלטת המנהל הכללי בדבר סירוב לבקשת רישיון, או ביטול רישיון תתויק יחד עם כל אלה (להלן – התיק):
(1)הבקשה לרישיון;
(2)טעמי ההחלטה;
(3)פרטים אחרים הקשורים בהחלטת המנהל הכללי לסרב לבקשה או בהחלטת המנהל הכללי לבטל רישיון.
