11. בקשות להענקת אות לבודדים
(א)בקשה להענקת אות מערכה לפי תקנה 5 יגיש המבקש אות המערכה עצמו ואולם רשאי השר להחליט על הענקת אות מערכה לפי התקנה האמורה אף בלא קבלת בקשה כאמור.
(ב)בבקשה להענקת אות יפורטו נסיבות העבודה או הפעילות כמתנדב של המבקש בתקופה הקובעת, ויצורפו לה מסמכים המאמתים אותן.
