Section 2 of כללי שירות המדינה (מינויים) (כללים ונוהל לקביעת כשירות רפואית), התשס״ט–2009, חובת המצאת סיכום מידע רפואי על ידי מועמד לשירות המדינה.
2. חובת המצאת סיכום מידע רפואי על ידי מועמד לשירות המדינה
(א)מועמד שנמצא מתאים להעסקה בשירות המדינה, יחל לעבוד לאחר שהמציא לאחראי במשרד סיכום מידע רפואי וטופס בדיקה ובו הצהרה מפורטת על מצבו הרפואי.
(ב)סיכום המידע הרפואי וטופס הבדיקה יועברו לצורך קביעת הכשירות הרפואית לרופא, אשר רשאי לפי שיקול דעתו להזמין את המועמד לבדיקה רפואית או לדרוש ממנו להיבדק בדיקות נוספות.
