22. הגשה של תביעה מהותית
הוגשה תביעה מהותית נגד החברה, יתואר בקצרה ההליך המשפטי, לרבות שם בית המשפט או המוסד שבו תלוי ועומד ההליך, אופי או סוג הצדדים המרכזיים בו, העובדות המהוות לכאורה את הבסיס להליך והסעד המתבקש.
