87. תוכן הבקשה וצירופיה
הבקשה תכיל –
(1)שם המבקש;
(2)מספר תיק בית הדין שבו נדון המבקש;
(3)נימוקי הבקשה;
(4)העתק מאושר מפסק הדין אליו מתייחס המבקש.
