4. הגשת תצהיר
המבקש זכות או המבקש מהרשות אישור לפי סעיפים 12 או 13 לחוק, יצרף לבקשתו תצהיר ערוך לפי הנוסח שבתוספת השנייה; לעניין בקשה לאישור לפי סעיפים 12 או 13 לחוק, אין בהגשת התצהיר לפי תקנה זו כדי לגרוע מחובת הגשת התצהיר לפי סעיף 4(ב)(7) לכללי משק החשמל (שעבודים והעברות שליטה), התשע״ט–2019, שהמבקש נדרש להגיש במסגרת הבקשה שמגיש לפי אותם כללים.
