2. התעודה
התעודה שהמוסד יעניק לנכה לפי סעיף 25א (להלן – התעודה) תכלול פרטים אלה:
(1)שמו ושם משפחתו של הנכה;
(2)מענו;
(3)מספר הזהות שלו;
(4)המספר הסידורי של הסמל והתעודה.
