17. הגשת תביעה לגמלה
תיקון: תשל״ד, ס״ח תשל״ז, תשנ״ח
(א)(בוטלה).
(ב)לשכת שירות התעסוקה [במקור: לשכת העבודה] תתן לתובע אישור לפי הטופס שבתוספת החל מ־5 לחודש בעד ימי אבטלה שקדמו לאותו חודש.
(ג)תביעה לגמלה לפי סימן זה תוגש למוסד פעם אחת בכל תקופה של שנים־עשר חדשים שלאחר התאריך הקובע כמשמעותו בסעיף 127ד לחוק.
