10. הגשת מועמדות להיכלל במאגר
מועמד המבקש להתמנות לתפקיד ממונה במועצה דתית יגיש בקשה באתר המשרד לשירותי דת להיכלל במאגר הכוללת את פרטי המבקש ודרכי ההתקשרות עימו וכן את המסמכים שלהלן:
(1)קורות חיים;
(2)מסמכים המצביעים על עמידת המועמד בתנאי הכשירות המפורטים בתקנות;
(3)הצהרה בדבר זיקה עסקית, אישית או פוליטית לשר משרי הממשלה;
(4)שאלון איתור ניגוד עניינים;
(5)המלצות אם יש.
