4. בקשה לרשיון
תיקון: תשמ״א
בקשה לרשיון תוגש למנהל, באמצעות הרופא המחוזי כמשמעותו בפקודת בריאות העם, 1940 (להלן – הרופא המחוזי), באזור מגוריה של המבקשת, ויצורפו לה שאלון לעובדים מקצועיים בשירותי הרפואה, העתק או צילום מאומת מהדיפלומה המעידה על השכלתה והכשרתה ביילוד, וכן תעודה המעידה על אזרחות ישראלית לפי סעיף 15 לחוק האזרחות, התשי״ב–1952, או תעודת משרד הפנים על מעמדה של המבקשת בישראל לפי חוק הכניסה לישראל, התשי״ב–1952.
