Section 3 of תקנות למניעת אלימות במשפחה (יידוע בידי אנשי דת וטוענים רבניים), התש״ע–2010, תיעוד פעולות היידוע.
3. תיעוד פעולות היידוע
(א)איש הדת או הטוען הרבני יתעד בכתב את הפעולות כאמור בתקנה 2 שעשה לביצוע הוראות סעיף 11א(ב) לחוק.
(ב)התיעוד לפי תקנת משנה (א) יהיה ערוך לפי הטופס שבתוספת, ויישמר בידי איש הדת או הטוען הרבני, לפי העניין, באופן, במקום ולתקופה שנשמר בידו שאר המידע על האדם.
