1א. טופס מסירת תלונה
תיקון: תשע״ב
(א)מסר מתלונן תלונה לממונה עליו, למבקר הפנימי במקום עבודתו או לגוף בודק, יציע מקבל התלונה למתלונן, למסור את פרטיו לפי טופס 1 שבתוספת, לשם קבלת תעודת ממצאים, אם תימצא תלונתו מבוססת.
(ב)מקבל התלונה יצרף את הטופס למסמך שערך בדבר התלונה וימסור למתלונן עותק של הטופס המלא.
