2. פרטי דיווח
(א)דיווח כאמור בתקנה 1(1) עד (3), ייעשה לפי הטופס.
(ב)דיווח טלפוני כאמור בתקנה 1(4) יכלול את הפרטים שבטופס, אשר שאל עליהם השוטר מהיחידה.
(ג)הטופס יהיה זמין לציבור בדרכים אלה:
(1)בתחנת משטרה או במקומות אחרים שימסור שוטר לפונה;
(2)במשלוח מן המשטרה, לפונה, לפי המען שמסר לשוטר, באמצעות פקסימיליה, דואר אלקטרוני או דואר רגיל, לפי בחירת הפונה.
