6. הגשת תביעה למענק מיוחד ולקיצבה מיוחדת
תיקון: תשל״ט
תביעה למענק מיוחד או לקיצבה מיוחדת על פי סעיף 69 לחוק תוגש בטופס שאפשר להשיגו בכל אחד מסניפי המוסד, ויצורפו לה:
(1)תכנית שהוכנה על ידי התובע בדבר השימוש במענק המיוחד לכל אחת מהמטרות המפורטות בתקנה 2(א)(2) בנפרד;
(2)ראיות הדרושות להוכחת התביעה. המוסד רשאי לדרוש מהתובע פרטים או מסמכים נוספים הנוגעים לתביעה בין לפני אישורה ובין לאחר מכן.
