4. חובת הודעה למוסד
התובעת תודיע למוסד בכתב –
(1)על המועד שבו הפסיקה בפועל לעבוד בשל שמירת ההריון;
(2)על המועד שבו חידשה עבודתה בתקופה שלגביה ניתן האישור הרפואי;
(3)על כל תשלום בשל שמירת הריון, שהיא זכאית לו על פי כל חיקוק והסדר אחר האמור בסעיף 103ו לחוק;
(4)על מועד סיום ההריון.
